Read more ... ustry prevent a crazed pilot in the first place? Why doesn't one of the most stressful jobs in the world have psychological screening and annual testing? "Self report?" that is the single most ridiculous thing I have heard in quite a while. Peer review and report is the second. There needs to be a psych screening at hiring and at least an annual testing and meeting with psychologists in the airline industry. Let'snot obsess about the door - let's obsess about why the airline let a mad man behind that door.
